Service Objectives
-
District Support Centre for Persons with Disabilities: by adopting a “district-based” approach, to provide one-stop community support services for people with disabilities and their family members or carers to enhance their caring capability. People with disabilities then have a better opportunity to continue living in the community.

Day Care Service for Persons with Severe Disabilities: to provide day care for persons with severe disabilities; to strengthen the caring capability of families or carers so as to enhance opportunities of persons with severe disabilities to continue living in the community.

Day Respite Service: to provide support and temporary daytime relief for family members or carers of persons with disabilities to take care of personal matters or emergence. This also helps temporarily relieve their caring duties and alleviate their stress.
Target Group

District Support Centre for Persons with Disabilities: people with disabilities living in the community; family members or carers of people with disabilities; and the community at large

Day Care Service for Persons with Severe Disabilities: people with severe disabilities aged from 15 to 59 living in the community

Day Respite Service: people aged 6 or above with disabilities who are living in the community
Scopes of Service

District Support Centre for Persons with Disabilities:
A range of individual and group training on household and community living skills are arranged by a professional team; people with autism or challenging behaviour are provided with clinical psychology service;
Outreaching support service is provided for hidden people with disabilities, including home-based individual treatment, home modifications and advice on purchasing of rehabilitation equipment;
Social and recreational development programmes and activities are organised;
Training on caring skills to carers of people with disabilities is provided, including education course, seminar, workshop, parent-child activities and so on;
Community education activities are organised with the participation of volunteers so as to enhance integration in the community;
Cases management service is provided, including need assessment, setting up of individual plan and counselling service, assisting people with disabilities and their carers to obtain resources in the community and making referrals to the appropriate services if necessary;
Set up resources corner to provide information about the community and drop-in service.

Day Care Service for Persons with Severe Disabilities: provide day care service including meal; nursing care and intensive personal care including assistance with daily living; maintenance programmes on basic living skills; and regular activities to meet service users’ social and recreational needs so as to enable them to maintain contact with the community.

Day Respite Service: provide temporary care support service at the centre and household during the daytime.

Fees
-
District Support Centre for Persons with Disabilities: Membership Fee: HK$21 per year; Family Membership Fee: HK$50 per year.

Day Care Service for Persons with Severe Disabilities: HK$60 per day (with lunch) or HK$988 per month (with lunch).

Day Respite Service: HK$20 per session (4 hours for one session without lunch), and HK$33 per hour for outreaching service.

Fees may be charged depending on individual activities.
Application Procedures

District Support Centre for Persons with Disabilities: apply directly to the centre, or referred by other social service organisations.

Day Care Service for Persons with Severe Disabilities: must be referred by social workers or medical officers.

Day Respite Service: apply directly to the centre, or referred by social workers at medical social services, integrated family service centres, special schools or rehabilitation social service organisations.
