Since October 18, 2021, the Health Bureau of the Government of the Hong Kong Special Administrative Region has awarded a service contract to the Neighbourhood Advice-Action Council to

operate the Islands DHC Express (I DHCE), providing community-based primary healthcare services to residents to raise awareness of chronic diseases prevention and to enhance citizen’s health management ability.

I DHCE provides healthcare services to the public, including Health Education, Disease Prevention, Health Risk Factors Assessment, and Health Consultations, aiming to reduce or prevent the onset of chronic diseases. Through Health Risk Factors Assessment or Screening Services, individuals with potential health risks are encouraged to have early detection and management of chronic diseases. For those already living with chronic illness (such as hypertension, high cholesterol, diabetes, lower back pain, OA knee, etc.), our Centre offers Patient Empowerment Programs which support and encourage them to build up a tailored self-management plan.

I DHCE also serves as a Community Health Resources Hub, networking of primary healthcare service providers in the community (including family doctors, allied health professionals, and Chinese medicine practitioners, etc.) to establish a community-based Healthcare Network. This network facilitates citizens in access health services more conveniently within their community and promotes early prevention of chronic diseases.
